Full exposure in the secondary survey is done to uncover occult injuries that clothing can hide, ensuring you don’t miss wounds, internal injuries, or posterior findings that aren’t visible otherwise. A complete visual and physical exam of all body surfaces, including the back and other areas covered by drapes, helps identify problems that would otherwise go untreated.

But exposing the patient also risks significant heat loss. Hypothermia during trauma care worsens bleeding and coagulopathy and is linked to higher mortality, so intentional warming is essential. Use active warming techniques—warm blankets or forced-air warming devices, keep the environment warm, and use warmed IV fluids—to prevent hypothermia while you conduct the exam.

So the best approach is to enable thorough detection of hidden injuries while actively preventing heat loss, addressing both the diagnostic need and the associated risk. The other considerations—documentation for legal reasons, or focusing on dehydration, or prioritizing imaging or sedation—don’t address the primary purpose and the main risk in the same way.
